Below are three PDF files regarding the donor egg process.
The first is detailed information on what you can expect if you are accepted and picked as an egg donor candidate.
The second PDF file is our application. Please print out and complete the donor egg application and mail it to: Val Gordon, Donor Egg Coordinator, South Jersey Fertility Center, 400 Lippincott Drive, #130, Marlton, NJ 08053. Make sure that you include a current photo of yourself. When selecting your picture please keep in mind that if you give us permission, your photo may be viewed by perspective candidates.
The last PDF is a letter which gives us permission to show your picture to a persepctive recipient. Please print out this consent form and pick either option #1 or option #2.
